Note:
New application refers to application without existing Streamyx at the same premise.
Migrate application refers to application with existing Streamyx at the same premise.


6. For existing Streamyx / TM Fixed-Line customer, pls provide

a.) Streamyx username
b.) TM Fixed-Line No.
c.) Address of current Streamyx / TM Fixed-Line

7. There are 4 options for existing Streamyx / TM Fixed-Line customer:

a.) Keep both services (not applicable for premise of 5 storey and above, which must terminate existing Streamyx)
b.) Terminate Streamyx & TM Fixed-Line
c.) Terminate Streamyx only
d.) Terminate TM Fixed-Line only (not applicable for Streamyx Customer, coz Streamyx is attached to TM Fixed-Line)

If applicant is non-existing customer (not owner of Streamyx / TM Fixed-Line), customer can do termination manually at TM Point.

8. Customer's email address is required for e-billing (or else, dealer's email will be used)

Customer can view their UniFi bill online. Printed bills will only be made available upon request at a fee of RM8 per month.

9. INSTALLATION

Deferment (within 24 hours of installation appointment time) is subjected to a charge of RM200.

Standard Installation

During promotion period, Standard Installation and Activation charges worth RM200 will be waived.

Additional charges beyond Standard Installation will be included in the bill (if additional cabling is required beyond what is provided in the Standard Installation.)

Non-Standard Installation

Customer might be charged few hundreds Ringgit for Non-Standard (Optional) Installation (e.g. wiring over ceiling, underground, conceal wiring, customised wiring, etc.)

These charges are to be negotiated and settled in cash with the contractor.

Before any work commence, customer will be required to sign-off on the Work Order Acceptance Form.

Customer will be responsible for providing TV and 4-socket extension cord to complete the installation.

Installation will take few hours.
